Your Student ID is your USERNAME and your birth date is your PASSWORD. Birth date is formatted m/d/yyyy with no preceding zeros for single digit months and days. Include the slashes and the 4 digit year.

FOR EXAMPLE: 

Your birthday is on February 5, 2007. February is the 2nd month. You would enter is as "2" not "02." You would then enter the day as "5" not "05." See format example here --> 2/5/2007
